Lumbar disc herniation is the leading cause of sciatica in working age population. Dr. NaPier performs a high volume of lumbar microdiscectomy surgeries in patients with disc herniations that do not respond to nonsurgical treatment. While lumbar microdisectomy is generally very successful, the biggest problem with microdiscectomy is recurrent lumbar disc herniation, which can occur in 7-15% of patients. The pathophysiology of a lumbar disc herniation is that the gelatinous cushioning material (nucleus pulposus) inside the disc ends up escaping through a crack in the fibrous outer layer of the disc (annulus fibrosis)

During lumbar microdiscectomy surgery the red herniated disc material is removed, but this often leaves a large defect in the fibrous crust of the disc (annulus fibrosis). This is especially common in young, active patients.
